Our person centred practice programs move your organisation and staff, from activities and routines developed up to 100 years ago, to what is accepted as best practice in 2014.
Age Concern Pty Ltd facilitates the change, supporting residents, families and staff, through our preceptor style programs.
An experienced facilitator is on-site to ‘walk’ your staff members through each of the person centred care practice programs.
We also review all documentation and offer suggestions on streamlining, up-dating processes and maximising funding.

Person Centred Practice
– Easy Start Three (3) Day Program
You identify one (1) resident for assessment and care development.
Step-by-step we advance the staff through contemporary assessment and documentation activities supporting the standards and ACFI We demonstrate how to develop interventions supporting person centred practice, incorporating behaviour management and those care changes that staff may have previously resisted.
This allows staff to experience person centred and evidence based practice in their own work place without being overwhelmed. This a ‘slow and steady’ start to Person Centred Practice.
We demonstrate how to modify the options for each of your residents, making this a self sustaining program. There are no extra staffing requirements
We licence your facility, for 12 months, to use the Standardised Mini Mental Status Examination – SMMSE (Molloy et al, 1991). You receive free, 100 copies of the tool and demonstrate how it aligns with PAS .
Program Format:
There are two (2) components to the program
Prior to visit:
we discuss with you the measurable outcomes that you would like to achieve.
you arrange for the Age Concern clinician to meet with the resident and or their representative; you arrange some short staff meetings, perhaps at handovers,
re-assess client as recommended
Visit
Day 1
The Age Concern Pty. Ltd. facilitator will review the health history, assessment data and psycho-social history of the resident and attend meetings as arranged.
Day 2
The facilitator will commence re-developing the care plan as staff members go about care delivery for the resident. This will include time with evening and night staff.
Day 3
Completion of the care plan and meetings with the resident, family and staff for discussion and approval/consensus of the final document.
Three weeks post program, you will receive a referenced report with details on how to progress in a self sustaining manner

Person Centred Practice
– ‘Kick Start’ Nine (9) Day Program
Suitable for general or special care units (approx. 30 beds)
The purpose of this nine (9) day program is to assist and support staff in the first step of person centred practice by revising one activity.
This is moving away from the morning routine of waking residents for breakfast. Most organisations are then comfortable to progress the change to all units and or sites.
Program Format:
Part 1. Introduction – two (2) days:
At least three weeks before the commencement of the program the following meetings are arranged:
- two (2) meetings for operators, owners, board members, management and senior department staff to discuss any particular issues, expectations, preparations and to identify and agree on outcome measures
- two (2) meetings for residents and or families to introduce person centred practicemixed meetings for staff from all departments, introducing the change and how it benefits staff as well as the residents, the issues. Evening and night meetings may be arranged.
Part 2. Contemporary Practice – six (6) days:
On the first day of the program the facilitator will be in the unit at 0500 hours to support staff as they experience their first day of person centred practice.
Includes one (1) weekend day
For the remaining four (4) days the facilitator will be ‘on the floor’ in blocks of time to have contact with as many staff as is possible and cover as many time frames during all shifts, as is possible. During this time all practices are observed, suggestions made as necessary and education and training if offered as necessary.
Assessment and documentation processes are reviewed with details of legal recommendations offered including the issue of the signing for medications.
Part 3. Review and Progress – two (2) days:
The review and progress is scheduled for eight weeks following the main change. During the first day the progress of the change is reviewed.
Meetings with resident, staff and family are arranged for the second day. Feedback regarding the change is requested and details of the success of the changes are discussed, including the measured outcomes. All should also be offered the opportunity to suggest directions for the future.

Three (3) Week Program
– Clinical Risk Management
This three-week program is useful for those organisations:
transforming an established unit to a centre of excellence
commissioning a new unit
requiring expert support to meet the aged care standards.
Program Format:
Part 1. Introduction – five (5) days:
At least three weeks before the commencement of the Part 2 of the program, the following meetings are arranged:
Day 1 and 2
- two (2) meetings for operators, owners, board members, management and senior department staff to discuss any particular issues, expectations, preparations and to identify and agree on outcome measures
- two (2) meetings for residents and or families to introduce person centred practice
- a series of one (1) hour meetings to be attended by as many staff as possible, from all departments discussing the change and the roles and issue for each department
Day 3, 4 and 5
A series of one (1) hour presentations for all owners, board members and all staff change inlcuding the issues; the reactions; possible coping mechanisms;
Evening and night meetings may be arranged.
Part 1 a The investment of education – five (5) days
At least one (1) week before the commencement of Part 2 person centred practice.
- Six (6) hours, face to face training Dementia and Behaviours for all staff – over five (5) days
- Post Graduate RNs Dementia program (30 + CPD hours) – no cost -all registered nurses – virtual classroom – commence after introduction (usually $880 per person)
- Dementia program ENs (15 CPD hours) – all ENs- virtual classroom – no cost – commence after introduction (usually $440 per person)

Assessment and Care Planning
This practice oriented program is about minimising documentation but maximising its effect for the individual residents and involves an Age Concern Pty Ltd staff member assisting your staff to become more efficient, effective and person centred in:
- Data collection
- Assessment processes
- Person centred care plan development and evaluation
This is not ‘selling’ you another Care Plan or package, but how to use yours effectively, and is a must if any of the following occur at your facility:
- PCA’s/AIN’s assist with assessments for high care residents
- You do not assess the cognition of residents within an hour of admission
- You care plan includes words such as repeat, assist, prompt encourage
- Your only behaviour record is the ACFI form
- You are not aware of the recent review of non-verbal pain assessment tools
Formats:
One Day Format (6 Hours)
You arrange a suitable date and time with Age Concern Pty Ltd and then forward to us (scan or mail) a set of your admission & assessment documents. With these forms in front of her, step by step, our facilitator, Prue Mellor, will show/discuss with you:
- How to maximise the use of each of your tools
- Additional tools that may be required, but not for funding
- How to translate PAS and Cornell findings into care strategies
- The issue with the Abbey pain tool, and
- How to re-word your care plans to substantiate your claims of person centred care and for funding
With the distance program there is an option to break the program into three sessions each of 2 hours.
Two Day Format (12 Hours)
Includes Day One (1), as above, plus Day Two (2) where we put the information together and, with you, develop a care plan for one of your residents based on your assessment data.
With the distance program there is an option to break the program into six sessions each of 2 hours.
