Person-Centred Dementia Care

Transforming Care from Routine to Truly Individualised Practice

Person-centred Dementia care

At Age Concern, we partner with residential aged-care services to deliver evidence-based, person-centred dementia care to exceed the NMBA and Aged Care Quality and Safety Commission standards—while respecting the unique needs and preferences of each resident.

We work ‘on the floor’ to support, empower and build the confidence of your staff as they transition to truly personalised care for each resident that minimises the behaviour symptoms of dementia.

Is Your Care Really Person-Centred?

Take a quick self-check — if your routines revolve around tasks instead of people, you may have the philosophy but not yet the practice. Click below to do the test.

Quick Test

Test your organisation for person centre practice (not just a philosophy)

  • Documented morning, afternoon and night routines for staff
  • Incontinence aids applied about 2100 hours
  • Televisions in the main lounge or dining room
  • Residents with Sundowner’s or being ‘intrusive’
  • Two-hourly turns and ‘major’ rounds about 0100 or 0500 hours
  • Shower and or other ‘lists’ and or shower residents alternate days

If you answered “yes” to even one — you may have the philosophy, but not the practice.

Additionally, the atmosphere within the MSU exudes a profound sense of tranquility and unhurriedness, allowing residents to focus on themselves and engage in purposeful activities, foster friendships, and spend quality time with one another. Staff members have reported that the unit has become easier to work in, and they have gained a better understanding of how to effectively manage certain behaviours.
